This file does not collect, send or store your data anywhere. Whatever you type stays in your own copy. Don't enter passwords, secrets or personal data you have no right to use. If you use it with real customer, employee or company data, you are responsible for keeping that data safe and for following the laws and agreements that apply to you.
Using the file
Designed for US Letter paper. On A4, print with “Fit to page.” Form fields work in Acrobat, Preview and most PDF viewers. Some viewers do not save what you type, so save a copy or print to PDF before you close it. Spreadsheets are tested in Excel-compatible software and should work in Google Sheets, but some formatting and charts can look different there.

This is Edition 1, 2026. Eddie Gear may update the material but has no obligation to. Using a later or earlier version is at your own risk.
